Job Title: Senior Community Mental Health Nurse – Dementia Service

Tripod Partners are currently recruiting for a Senior Community Mental Health Nurse to join the Dementia Service in Shropshire. This role focuses on the assessment and diagnosis pathway within the service.

Key Responsibilities

You will support the management of referrals into the Dementia Service, working closely with the operational lead and Nurse Practitioners to ensure efficient processing.

Your role will involve reviewing referral information, gathering additional details if necessary, and coordinating with primary care, other services, patients, and their families to ensure smooth progression to assessment.

This position requires experience in dementia care, particularly in assessment and diagnosis, with a preference for experience in dementia or older adult services.

The role allows for a blended working approach, with a mix of home-based work and some office presence, particularly during the initial training and induction phase.

Position Details:

* Location: Shrewsbury

* Work Pattern: Monday to Friday, 09:00-17:00 (full-time, 12-week contract)

* Rates:

* Day Rate: £27.00 per hour

* Saturday Rate: £32.00 per hour

* Sunday Rate: £38.00 per hour

* Pay Type: Umbrella or PAYE available

Requirements:

* Experience in dementia care, with a focus on assessment and diagnosis

* Preferably experience in a dementia service or older adult setting
