Working under the supervision of registered nurses to deliver a high standard of inpatient care in partnership with the wider clinical team, the service users and their family members/carers.
To support the named nurse/primary nurse in the assessment of needs for a group of patients and deliver their care under supervision.
To work within a team of nurses and other professionals who are committed to the delivery of a quality service.
To adopt a flexible approach to hours of duty, in order to meet the needs of the service over a 24-hour period.
To be an active participant in the multi-disciplinary team, assisting in the formulating of care plans and utilising skills to inform patient assessments and evaluations.
To contribute to the information recorded on the patient information system by reporting to nursing staff on patient’s presentations throughout the shift.
Under the supervision of the registered nurses, to undertake assessments of individuals and their families and carers as appropriate admitted to inpatient care, including service users presenting with high risk.
Under the supervision of the registered nurses, to develop care plans and discharge plans in collaboration with the service user and their families and carers
Under the supervision and guidance of the registered nurses, assess patient’s risks against their risk management plans and manage risks accordingly.
Under the supervision of the registered nurses regularly review risk factors and provide observational information to the MDT to enable them to modify care plans.
To support the Primary Nurse and Named Nurse for each shift in the provision of care to a group of inpatients.
To ensure that you are familiar with, and work within our policies and procedures at all times.
Working with the education department, complete the designated care certificate and level 1 diploma in dementia care within the timescales given to you.
Working under supervision, to confidently, competently, sensitively and safely, give care to a group of patients
To maintain confidentiality in accordance with the Information Governance polices at all times.
To be familiar with the safeguarding policy and procedures, to take appropriate action when working with vulnerable adults and children by liaising with the nurse in charge/safeguarding lead and advising of concerns.
To greet, accompany and assist professionals from outside the hospital who are visiting patients and liaise with nurse in charge as required.
Being able to act promptly and safely in recognising patients’ needs and reporting changes to the nurse in charge.
Collect data as directed to ensure service delivery is accurately captured, recorded and reflected.
Ensure participation in both managerial and clinical supervision, to assist with your own professional development and the maintenance of care standards.
Ensure that own development needs are addressed through the staff appraisal framework.
To be familiar with the impact of people detained under the MHA or DOLS/LPS) and be aware of your responsibilities at all times.
To promote respect for diversity and combat stigma and exclusion at every opportunity.
To communicate with and support relatives and carers and actively promote their involvement with the patients’ care plan.
