Job Title: Band 6 Community Nurse – Dementia Service
Location: Shrewsbury, Shropshire
Contract: Full-Time (Mon – Fri, 09:00 – 17:00) | 12 weeks

Job Summary:

The Dementia Service in Shropshire is seeking a Band 6 Community Nurse for a 12-week assignment. You will support the Assessment and Diagnosis pathway, managing referrals and collaborating with Nurse Practitioners to gather necessary information for dementia assessments.

This role offers a blend of home-based and office work, with office time needed for training and induction.

Key Responsibilities:

* Manage and review referrals for dementia assessments.

* Coordinate information gathering from primary care, services, patients, and families.

* Work closely with multidisciplinary teams to ensure timely, thorough assessments.

Requirements:

* Registered Nurse with experience in dementia care (assessment and diagnosis focus).

* Strong communication and organizational skills.

* Experience in dementia services or older adult care preferred
