The London Borough of Islington is seeking a talented Learning Disabilities Social Worker to join our dynamic team. This role offers a unique opportunity to work with a diverse client group, providing essential support and advocacy.

Key Responsibilities:

* Undertake comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.

* Develop and implement person-centered care plans that promote independence and well-being.

* Provide support and guidance to individuals and their families, addressing complex challenges and promoting positive outcomes.

* Collaborate with multidisciplinary teams, including healthcare professionals, housing providers, and other relevant agencies.

* Advocate for the rights and needs of individuals with learning disabilities, ensuring they have access to appropriate services and support.

* Maintain accurate and up-to-date case records, adhering to professional standards and legal requirements.

Essential Requirements:

* Social Work qualification (Degree or Diploma level)

* Registration with Social Work England

* Proven experience working with individuals with learning disabilities

* Strong assessment and care planning skills

* Excellent communication and interpersonal skills

* Ability to work independently and as part of a team

* Commitment to promoting person-centered practice

* Flexibility to work outside of traditional office hours as required
