About the Role:
We are currently seeking a motivated and experienced Band 6 Specialist Physiotherapist to join the Physiotherapy Team at Christchurch Hospital's Day Hospital. This role plays a key part in admission avoidance, community-based rehabilitation, and outpatient care for elderly and frail individuals.

Key Responsibilities:

Deliver high-quality physiotherapy interventions in both 1:1 and group formats

*

Work closely with multidisciplinary teams to support patients in maintaining independence

*

Assess and manage a caseload of patients with varying and complex needs

*

Contribute to service development and ensure best practice in physiotherapy care

*

Support patients through the rehabilitation pathway, aiming to prevent unnecessary hospital admissions

About You:

*

HCPC registered Physiotherapist

*

Experience working in elderly care, frailty, or community rehabilitation

*

Excellent communication and interpersonal skills

*

Ability to work both independently and as part of a multi-disciplinary team

*

Strong clinical reasoning and problem-solving skills
