Job Title: Social Worker – Adults Community South Team

Pay rate: £33 - £35 (depending on experience)

About the Role:
Bristol City Council is seeking an enthusiastic and compassionate Social Worker to join our Adults Community South Team. This role involves working closely with adults in the community who may have physical disabilities, learning difficulties, or mental health needs, ensuring they receive the necessary support to live independently and with dignity. Key Responsibilities:

* Conduct needs assessments and reviews under the Care Act 2014, ensuring that individuals are supported in a person-centred, strengths-based way.

* Develop and implement care and support plans that promote independence and well-being.

* Collaborate with a range of professionals, including healthcare providers, housing officers, and voluntary organisations, to coordinate services and interventions.

* Provide safeguarding support and manage risk to ensure vulnerable adults are protected from harm and abuse.

* Work in partnership with individuals, their families, and carers, empowering them to make informed decisions about their care.

* Ensure all case records are maintained to a high standard and are in line with statutory requirements.

* Participate in multi-disciplinary meetings and contribute to case conferences as required.

* Promote a culture of equality, diversity, and inclusion in all aspects of your work.

Essential Requirements:

* Degree in Social Work (or equivalent qualification).

* Registered with Social Work England.

* Experience working with adults in a community-based setting, including managing complex cases.

* Strong knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.

* Excellent communication, assessment, and decision-making skills.

* Ability to work collaboratively with individuals, families, and partner agencies to deliver positive outcomes.

* Full UK driving license and access to a vehicle for community visits.

Desirable:

* Experience in a local authority adult social care team.

* Knowledge of Bristol’s local services and community resources.

* Experience working with adults with learning disabilities or mental health needs
