Proven ability to produce high-quality assessments and reports.NonStop Consulting are seeking experienced and qualified Social Workers to work collaboratively with health colleagues on retrospective Continuing Healthcare (CHC) cases. This is a unique opportunity to contribute your expertise to a critical area of social care, ensuring compliance with CHC processes and supporting individuals and their families with sensitivity and professionalism.
Key Responsibilities
Collaborate with health colleagues to assess and review retrospective CHC cases, ensuring alignment with established procedures and legal frameworks.
Demonstrate sound knowledge of CHC Decision Support Tools, the London Health Needs Tool, and relevant legislation.
Undertake comprehensive social care assessments and produce detailed support plans or reports tailored to individual needs.
Engage sensitively and effectively with families of deceased service users to provide necessary support and information.
Manage casework across two locations/sites, ensuring accurate documentation and timely updates on multiple databases.
