Your main responsibilities

* Facilitating hospital discharge

* Carrying out assessments in different wards.

* Managing a busy caseload within an acute hospital setting.

* Working closely with health colleagues.

* Ensuring adults in hospital are discharged in a timely and appropriate manner.

* Undertaking section 2s and 5s.

* Attending multi-disciplinary meetings.

* Mental capacity assessments leading to best interest decisions.

* Advising patients and families who do not meet the eligibility criteria and redirect them to other agencies.

* Advocate for the rights and entitlements of individuals, ensuring they have access to the necessary support services.

* Maintain accurate, timely, and up-to-date records of assessments, interventions, and outcomes in compliance with organisational and legal requirements.

* Stay informed about relevant legislation, policies, and best practices in social work and hospital discharge processes
