Northamptonshire Children’s Trust was established in November 2020 to deliver Children’s Services to the newly established North and West Northants County Councils. The Trust is wholly owned and funded but operationally independent from the Councils; this provides the organisation with unique opportunities to explore new ways of working and to be more agile and adaptable in its approach to service delivery. Job Duties & Responsibilities:
* Conduct Initial, Core, and specialized assessments as per national guidelines to create support plans for service users and coordinate their implementation or refer cases appropriately.
* Manage complex cases, organize support packages, assess risks to health and wellbeing, and monitor and review situations to adjust support as needed.
* Work directly with family members to provide advice, guidance, and empower service users to promote participation in planning and evaluation of services, increasing self-sufficiency and independence.
* Liaise with and take a professional lead in involving appropriate staff and agencies to meet service users' needs and establish resources like group work programs and specialist counseling.
* Attend and participate in multidisciplinary teams and meetings to analyze service users' needs and plan care programs accordingly.
* Ensure efficient and timely recording, updating, and completion of all relevant information and reports per policies and procedures for audit, court, or other legitimate purposes.
* Provide written/verbal evidence in court/adoption/fostering panels as required, plan and assess related proceedings, and deliver evidence to support decisions made by social services to safeguard children at risk.
* Implement court-agreed actions to ensure service users' safety and wellbeing, including the potential removal of children from their homes and placement in appropriate care or foster homes.
