Social Worker (Adults) - Illford

We are seeking a Social Worker (Adults) - Illford to join our Adults Social Care team. The primary responsibility will be to work on the the discharge to practise model, coordinating services for the recommended discharge pathway to meet the assessed needs of patients prior to discharge from the hospital.

Rate: £22.47 (PAYE), £28.33 (Ltd)

Key Responsibilities (Social Worker (Adults) - Illford):

Arrange services for the recommended discharge pathway to meet the assessed needs of patients prior to discharge from hospital.
Undertake reviews of patient needs and care plans.
Ensure knowledge of relevant legislation required to implement the responsibilities of local authorities.
Conduct assessments as required.
Collaborate with healthcare professionals and other stakeholders to facilitate smooth transitions for patients.

Requirements (Social Care):

HCPC Registration
3 years of work history (1 year must be in relevant field).
Eligibility to work in the UK.
