Bristol City Council are currently recruiting for an adult Social Worker to join their triage and response team based at Southmead Hospital and the Bristol Royal Infirmary hospital. This role is for a full-time locum working 37 hours a week.

The Role:

To enable service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support.

This role will include completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation, developing comprehensive discharge plans, and collaborating with medical staff to ensure seamless patient care transitions.

Essential Requirements:

* Social Work Qualification
* Experience as a social worker within adult social care
* Social Work England Registration
* Driving license and access to own vehicle
