London Borough of Lambeth are currently recruiting for a qualified adult Social Worker to join their Adult Transitions team. This is a full-time locum role.

The Role:

The role is for an adult transitions team, Undertaking Care Act assessments and associated work with young people transitioning into Adult services from Childrens services

Essential Requirements:

* Social Work Qualification - Degree
* Experience as a Social Worker working with transitions teams
* Social Work England Registration
