Social Worker

Long term team

Duties :

Regularly assess the needs of individuals to determine the level and type of support required.
Create detailed care plans that outline the services and support necessary to meet the long-term needs of the individual.
Regularly review and update care plans to ensure they continue to meet the changing needs of the individual.
Manage a caseload of individuals, ensuring they receive continuous and appropriate services.
Work with other professionals (e.g., health care providers, housing officers) to coordinate services that support the individual's well-being.
Ensure that the voices of individuals and their families are heard, particularly in decision-making processes.
Identify and assess any risks to the safety or well-being of the individual.
Put in place appropriate safeguarding measures to protect vulnerable individuals from abuse or neglect.
Report any concerns about abuse or neglect and take necessary actions in collaboration with relevant authorities.
Keep detailed and accurate records of assessments, care plans, reviews, and any other significant interactions.
Participate in regular supervision to reflect on practice, receive guidance, and manage emotional stress associated with the role.

Experience and Requirements :

Social Work degree
2 years post qualifying experience
