Spencer Clarke Group are seeking a Short Breaks Practitioner for a Local Authority Client based in North London.

In this role, you will lead the coordination of short breaks and social care packages for children and young people with disabilities.

About the role:

You will be tasked with delivering key strategic objectives:

Act as the lead professional and care coordinator for a caseload of disabled children with complex needs.
Conduct person-centred short breaks assessments and reviews of social care packages.
Collaborate with internal teams and external agencies to deliver holistic, coordinated support for children and families.
Provide advice to families about services and benefits, supporting them in decision-making processes.
Prepare reports and recommendations for resource allocation and present them at Panels.
Maintain accurate case files, records, and documentation in line with departmental practice.

About you:

The successful candidate will have the following skills / experience:

Experience working with children with disabilities and their families.
Strong knowledge of relevant legislation, including the Children Act 1989, Children and Families Act 2014, and SEND framework.
Experience in conducting assessments of individual needs, creating care plans, and reviewing those plans.
Experience supporting families in decision-making and navigating available services and benefits.

What's on offer:

Salary: £20+ph

*may negotiate higher for exceptional candidates, based on experience*

Contract type: 3-6, months minimum, with a high likelihood of extension

Hours: Monday to Friday, 37 hours per week
