Key Responsibilities

Coordinate care for residents, focusing on their strengths and community networks.
Conduct quality conversations, triage, initial assessments, support planning, and case coordination.
Collaborate with social workers, occupational therapists, and support staff to ensure seamless service delivery.
Work within legal frameworks, safeguarding principles, and strengths-based practices to optimize outcomes.
Promote independence by connecting residents with voluntary and community groups.About You
We are looking for individuals with:

A QCF Level 3 qualification in health and social care (or willingness to achieve it).
Experience in social care, health, or the voluntary sector.
Strong knowledge of legislation and guidance related to adult care.
Excellent communication, negotiation, and advocacy skills.
A proactive, solution-focused approach to challenges.
