Salary: £42,861 - £50,646 per annum

The Role:

As a Social Worker in CIS, you will:

Conduct Assessments: Assess needs at home and in community settings, developing person-centred care plans.
Work Collaboratively: Team up with Occupational Therapists and Independent Living Assessors to deliver integrated care.
Safeguard and Support: Ensure service users are protected, and provide support to families and carers.
Proactively Plan: Look ahead to prevent future needs and create sustainable support options.You will document assessments and work with a multidisciplinary team to deliver meaningful impact.

About You:

You will be:

A qualified and registered Social Worker with experience in community settings.
Experienced in Mental Capacity Assessments and creating care plans.
Familiar with assistive technologies and safeguarding procedures.
Competent using Mosaic, System1, and MS Office.You will need an enhanced DBS check and a passion for delivering high-quality care to diverse communities.
