Somerset Council are currently recruiting for an adult Social Worker to join a hospital discharge team at Bath Royal United Hospital to support discharge planning. This role is for a full time locum working 37 hours a week and will be based on site daily.

Essential Requirements:

* Social Work Qualification
* Experience as a social worker within adult social care
* Social Work England Registration
* Driving license and access to own vehicle
