Responsibilities: 
Conducting detailed occupational therapy assessments for patients in their own homes or community settings.
Identifying the needs, goals, and preferences of individuals and developing personalized care plans to improve daily living activities (ADLs) and independence.
Recommending adaptive equipment and modifications to the home or work environment (e.g., grab rails, ramps, specialized furniture) to increase safety and independence.
Supporting individuals recovering from surgery, illness, or injury by helping them regain functional abilities and adapt to new limitations.
Attending multidisciplinary team meetings to discuss care plans and make recommendations for treatment.Qualifications and Requirements:
Fully enhanced valid DBS
Must have Occupational Therapy Qualification  
A minimum of 12 months post qualification experience is desired.Benefits:
