Location: IG11
Working Hours Monday - Friday 9am - 5pm
Duration: No longer than 3 months
Responsibilities: Providing the Health Child programme for 0-5 children and families. Supporting safeguarding, attending CP conferences, core groups, early help and mentoring, home visits and clinics for Universal Plus and Partnership Plus
Skills: Qualified HV experience with the use of Rio using the templates, good communication and able to support changes. Require a work risk assessment
Overview
The Health Visitor plays a crucial role in promoting health and wellbeing within the community, particularly for families with young children. This position involves working closely with parents and caregivers to provide guidance, support, and education on health-related matters. The ideal candidate will possess strong communication skills, time management abilities, and a background in nursing or a related field.
Duties
* Conduct home visits to assess the health and developmental needs of children and families.
* Develop and implement individual care plans tailored to meet the specific needs of families.
* Provide education on health promotion, parenting skills, and child development.
* Collaborate with other healthcare professionals to ensure comprehensive care for families.
* Maintain accurate records of assessments, interventions, and outcomes.
* Lead community health initiatives to raise awareness about available services and resources.
* Support families in accessing additional services as needed, including mental health support and social services.
Requirements
* Registered Nurse qualification with experience in community health or paediatrics preferred.
* Strong leadership skills to guide families through health challenges.
* Excellent communication skills to effectively engage with diverse populations.
* Proven time management abilities to manage multiple cases efficiently.
* Familiarity with developing care plans that address individual family needs.
* A compassionate approach with a commitment to promoting the wellbeing of children and families.
