Job Title: Practice Nurse
Location: Salisbury, Wiltshire

Job Summary:
A well-established and friendly GP practice in Salisbury is seeking a dedicated and compassionate Practice Nurse to join their dynamic healthcare team. The successful candidate will play a crucial role in delivering high-quality care to a diverse patient population, providing a range of clinical services in a supportive and professional environment.

Key Responsibilities:

Conducting patient assessments, including chronic disease management, wound care, immunizations, and family planning services.
Performing routine health checks, such as blood pressure monitoring, ECGs, and health screenings.
Administering vaccinations and injections, including travel immunizations and childhood vaccines.
Supporting patients with long-term conditions, providing advice and management for conditions such as diabetes, asthma, and hypertension.
Assisting in the development and implementation of individualized care plans.
Offering lifestyle advice to patients, including smoking cessation, weight management, and healthy living.
Collaborating with GPs and other healthcare professionals to ensure integrated and coordinated patient care.
Maintaining accurate and up-to-date patient records in line with practice policies and procedures.
Participating in clinical audits and contributing to the continuous improvement of services.

Qualifications and Experience:

Registered Nurse (RGN) with current NMC registration.
Previous experience working in a primary care or general practice setting is desirable.
Proficient in chronic disease management, wound care, and immunizations.
Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
A commitment to delivering high standards of patient care and promoting health and well-being.
Ability to work independently as well as part of a multidisciplinary team.
Familiarity with electronic patient records and IT systems used in healthcare.
