About the role

Based within the Personal Development Team, this role is responsible for the service delivery of an effective, high-quality, person-centred support service to residents with a range of multiple needs.

They will be the principal providers of support to residents in the Core service to build hope, enable lasting change; supporting residents to instil independence and long-term sustainability, to ensure we mitigate as much as possible, a return to homelessness.

The role will require shift work, including evenings and weekends.

Salary: £28,080 - £32,240 annual salary
Contract: Permanent
Hours: 37.5 Monday to Sunday
Location: Canning Town

Responsibilities:

* Responsible for day-to-day delivery of the Core service, reporting to the Team Manager and Deputy Team Manager.

* Apply a positive, trauma-informed approach with residents who have complex needs and challenging behaviors.

* Use motivational interviewing and asset-based techniques to assess and create support plans and risk assessments for residents.

* Support residents from service entry, helping them with social reintegration and leading purposeful lives.

* Conduct regular reviews of support plans and risk assessments to meet changing resident needs.

* Take a proactive approach to coordinating multi-disciplinary support for residents.

* Work within a Psychologically Informed Environment (PIE) and strive to meet service targets and deliver results.

* Ensure a high standard of customer service and professionalism is upheld.

* Attend team meetings and contribute to service policy and planning discussions.

* Stay updated on housing and welfare legislation relevant to supporting and moving residents on.

* Process and respond to all referrals, ensuring they meet service eligibility criteria.

* Ensure casework is accurately recorded, and all service outcomes are entered into the case management system.

* Collect and report follow-up information regarding reconnection outcomes.

* Address immediate support needs of residents, ensuring timely intervention.

* Assess resident safety and develop risk management strategies in collaboration with other services.

* Adhere to and implement decisions made by the management team.

* Maintain accurate records on referrals, service outputs, and outcomes.

* Keep financial and administrative systems, including resident files, up to date and in line with policies.

* Ensure all assessments and service outcomes are promptly entered into the case management system.

* Liaise effectively with external agencies, including the police, social services, and voluntary organizations.

* Build and maintain positive working relationships with colleagues to foster team cohesion.

* Ensure compliance with all policies, procedures, and health and safety regulations.

* Perform additional duties within the post holder's competence as required.

About You:

* Proven experience working with individuals facing homelessness or rough sleeping.

* Strong understanding of issues such as substance misuse, antisocial behavior, and offending.

* Experience working in a trauma-informed way with proven effectiveness.

* Understanding of statutory compliance, including health and safety, equal opportunities, and data protection.

* Demonstrated ability to motivate residents with support needs to engage in meaningful activities.

* Ability to work collaboratively with colleagues and across agencies to achieve shared goals.

* Excellent organizational skills, able to manage tasks and meet deadlines with minimal supervision.
