The role: Social Care Assessor - Adult Social Care Team

Location: Liverpool

Rate of Pay: £22 Per Hour

Are you an experienced Social Care Assessor with a strong understanding of legislation and a practical approach to supporting fast-paced Adult Social Care Teams?

We are seeking a Social Care Assessor to join the Adult Access Team based at the Cunard Building. This is a secondment cover role, offering a unique opportunity to make a meaningful impact by working with adults aged 18 and over.

Key Responsibilities:

Conducting detailed Reablement Assessments and support plans.
Carrying out Care Act Assessments and Mental Capacity Assessments.
Supporting adults with varied and complex needs, including hoarding, substance misuse, mental health, older people, and learning disabilities.
Assessing eligibility for ongoing community support and providing reablement services to promote independence.
Working proactively to prevent, reduce, and delay the need for care and support.Essential Skills:

Proficiency in Liquid Logic case management software.
Ability to manage a varied caseload efficiently in a high-pressure environment.
Strong knowledge of Adult Social Care legislation and best practices.
