About the Role:
As a Community Health Care Support Worker, you will play a vital role in supporting individuals to live independently in their own homes. You will provide compassionate care tailored to each individual's needs, ensuring their well-being, dignity, and independence.

Key Responsibilities:
✔ Providing personal care, including washing, dressing, and toileting
✔ Supporting individuals with mobility and medication administration
✔ Assisting with meal preparation and household tasks
✔ Encouraging independence and engagement in social activities
✔ Monitoring health conditions and reporting any concerns
✔ Working closely with families and healthcare professionals to provide person-centred care

What We're Looking For:
✅ Full UK driving licence and access to a vehicle
✅ Experience in community work, domiciliary care, or supported living
✅ NVQ Level 2 or 3 in Health & Social Care (preferred but not essential)
✅ A caring, patient, and professional approach to support work
✅ Ability to work independently and as part of a team
