Social Worker - Hospital Discharge Assessment Team

Up to £32 per hour

Location: Kings Mill Hospital, Sutton-in-Ashfield

About the Role:

We are seeking a skilled and experienced Social Worker to join our Hospital Discharge Assessment Team. You will play a crucial role in facilitating timely and safe discharges from Kings Mill Hospital.

Key Responsibilities:

* Conduct comprehensive assessments of patients' social and care needs upon hospital admission.

* Develop and implement discharge plans, coordinating with multidisciplinary teams to ensure seamless transitions of care.

* Liaise with community services, care providers, and other relevant agencies to secure appropriate post-discharge support.

* Advocate for patients' needs and rights, ensuring they receive the best possible care.

* Participate in a rota system to cover the Transfer of Care Hub at Kings Mill Hospital.

* Maintain accurate and up-to-date records.

* Promote effective communication and collaboration with healthcare professionals, patients, and families.

Essential Requirements:

* Social Work Degree (or equivalent)

* HCPC Registration

* Significant experience in a hospital or community setting

* Strong assessment and care planning skills

* Excellent communication and interpersonal skills

* Ability to work independently and as part of a team

* Flexibility to work shifts, including weekends and evenings

Desirable Requirements:

* Experience in discharge planning and complex case management

* Knowledge of local health and social care services

* Ability to manage a demanding workload
