Princess Alice Hospice is one of the top ten adult hospices in the UK, providing specialist palliative care and support to a community of more than a million people living in Surrey, south west London and Middlesex.
About the Hospice at Home team:
Our Hospice at Home service brings the skills and expertise of our highly trained and dedicated specialist team directly into our patient’s homes. This allows patients, their families, and carers access to the vital support they need at home, when time matters most.
About the role:
We are looking for an experienced and versatile nurse to work primarily within our Hospice at Home Service as well as, to support all clinical areas on a day to day basis.
You will work closely with our Enhanced Support Service and ensure timely pathways for patients supporting discharge from hospital.
You will be able to respond to referrals where an urgent visit is required to support patients and families in crisis. In this way more of our patients will be able to achieve their preferred place of care/death.
We also have the opportunity of a training post over a six month to twelve-month period to develop into the role of a Rapid Response CNS if more relevant. This role would start as a Band 6, with a review of the banding to be considered after the training.
About you:
You will have excellent communication and assessment skills and be able to work in an emotionally demanding environment. You will demonstrate leadership capabilities and enjoy working alongside a multidisciplinary team to assess the needs of patients, families, and carers.
You will have a relevant degree, diploma or module at masters level or in the process of undertaking one. Palliative care or oncology experience is essential.
