Job Opportunity: Outreach Worker (Part-Time)

P4 Recruitment is delighted to advertise this fantastic opportunity on behalf of our client, who is seeking a dedicated Outreach Worker to join their team.

Hours: 21-28 hours per week (flexible distribution of hours, consistent weekly schedule required)
Location: Various locations, including home visits and hospital settings across Wokingham
Essential Requirements: A valid driving licence and access to a car

Key Responsibilities:

* Conduct home visits to clients with limited mobility to complete assessments and provide key support.

* Visit clients who are off script/not in treatment to attempt re-engagement, often in collaboration with another professional.

* Offer ongoing support to service users in hospital settings when appropriate.

* Provide onsite assessments, duty cover, and clinic support as required.

* Collaborate with partner agencies to deliver comprehensive support.
