Belmont Recruitment are currently looking for an experienced Community Care Officer to join Lincolnshire Council's Adult Social Care Team on an initial 3 month temporary contract. This is a full-time role working 37 hours per week on a shift basis.

The Role:

We are a team lively of Community Care Officers and Social Workers based within Grantham District Hospital. The role is based on site to support strengths based conversations with the people that we support.

You will be responsible for having strengths based discussions and undertaking assessments with our customers to make use of their informal support networks. You will work as part of the 7 day Adult Care rota.

This equates to approximately 1 in 6 weekends worked, and these are all scheduled 3 months in advance.

Requirements:

* Experience of identifying needs and tailoring care and support to address needs.

* Knowledge of legislation relevant to the provision of health and social care

* Experience of working within Adult Social Care
