This is a fantastic opportunity for a passionate and experience Practice Nurse to join a highly reputable GP Training Practice for 3-4 days per week. Benefit from working alongside a multidisciplinary team renowned for providing first class service to patients. The practice is very high performing with CQC and QOF and this is a fantastic opportunity for someone experienced with routine Practice Nurse duties, as well as experience with Chronic Disease Management.  

Salary – £35,000 - £38,900 per annum DOE FTE (£17.95-£19.95 per hour) + NHS pension + 27 days annual leave + Bank Holidays + Employee Assistance Program

Your role –  

Essential experience as a Practice Nurse in Primary Care comfortable with all routine Practice Nurse responsibilities and at least one or two diplomas in Chronic Disease Management. 

Support with career development. 

Need to be experienced with SystmOne. 

Benefit from a very supportive MDT around you. 

3-4 days per week. 

20 minute paid coffee breaks during the morning and afternoon sessions.
