An exciting opportunity has arisen for a qualified Senior Social Worker to join Cambridgeshire County Council’s Continuing Healthcare team for an initial 3-month contract.

The successful candidate will Work in partnership with Cambridgeshire and Peterborough Integrated Care Board (ICB) and determine individual’s eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process.

The role will require you to lead on practice and performance development through the provision of high quality supervision and support of staff in line with The Care Act 2014 principles. You will take a lead on highly complex cases, working creatively in partnership with others. You will also provide leadership, through managing essential functions of the team including complex cases.

The role involves completing Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals needs and supporting the Multidisciplinary teams’ recommendation on eligibility for NHS Continuing Healthcare. Where individuals are not eligible for NHS Continuing Healthcare but are eligible for care and support from Cambridgeshire County Council in line with the Care Act 2014, the successful candidate will implement this.

Hybrid - Required to come into the office which is at Scott House, 5 George Street, Huntingdon two days per week.
