Job Title: Practice Nurse

Job Summary:

A well-established and patient-focused medical practice in Leicester is seeking an experienced and dedicated Practice Nurse to join their dynamic healthcare team. The successful candidate will play a vital role in delivering high-quality care to a diverse patient population.

Key Responsibilities:

• Provide comprehensive nursing care, including chronic disease management, wound care, immunizations, and health screenings.
• Conduct patient assessments, develop care plans, and support the delivery of patient-centered care.
• Collaborate closely with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure continuity of care.
• Offer patient education and health promotion advice, empowering individuals to take control of their health and well-being.
• Maintain accurate and up-to-date patient records, ensuring compliance with all relevant guidelines and protocols.

Qualifications and Experience:

• Registered Nurse (RGN) with a valid NMC pin.
• Previous experience in a primary care or community setting is highly desirable.
• Proficient in chronic disease management, including diabetes, asthma, and COPD.
• Strong communication skills and the ability to work effectively within a team.
• Competence in using clinical systems and electronic patient records.
